Chuck - Loudness also depends on what the venue will allow, if your in a club in the same area as Poker Machines and they ask you to keep the noise at a reasonable level then to turn it up would be a case of biting the hand that feeds you. I've worked in rooms with noise limiters that are wired into the power outlets that you have to use, get the Red light happening for 6 seconds continuously and the result is you loose power to all your gear. Which is not only a pain in the rear end as you have to turn everything off at the equipment switches or you can damage equipment if you don't. Let me tell you it's a nightmare watching that red light all night. By the way the levels for setting the sound limiter's (which are adjustable) are normally located behind the bar, so upset management and they can make your life misery and the punters soon get pissed off with being cut off mid song, result they go somewhere else. If in my opinion the sound is loud enough for the venue we are playing in and a new punter gets up and tell's me to crank it up I just inform him 'you sing, I mix, I promise you you'll sound great'

Clarity with reasonable volume to me is the aim, placemant of speakers and learning your regulars needs when it comes to mixing so they feel comfortable and enjoy what they are hearing when they are singing also helps a lot. I don't use foldbacks as such I'll position a FOH speaker so that the singers and I can hear exactly same mix and volume of the mix as what the rest of the audience is hearing, it works for me.
