Jason: Sure, although I don't join in the Whirlpool discussions due to work reasons, I am impressed by Whirlpool and the annual survey. Some ideas could be garnered from karaokescene.com as well. I'd gather the survey questions into groups:

* Questions on locality and transport.
How important is public transport access, how far would you travel during the week vs the weekend?
Which station or routes do you prefer [eg I'm iffy on Bankstown and beyond late at night]

* Questions on venue features.
What style of venue do you prefer [RSL, Pub, Music venue]
What type of crowd [ hens night, small local crowd, large rowdy party crowd] and age
Food, pool tables, juke box, beer & wine range

* Questions on hosting
Do you prefer dead air, banter or music between songs
Do you prefer a laid-back hosting style or an energetic style?

* Questions on songs and equipment
Nominate your top 10 songs in the order you sing them
Preference on microphones [corded, cordless, studio]
Mixing [eq, bass, distance you hold the mic]

With the optional nomination of age group and area that you live and area that you work as I suspect the answers will cluster based upon age and location. I wouldn't try dreaming up all the questions upfront but have a rolling mini-survey of say 5 questions to save '50 questions oh my god' symdrome.
